Our signature forms can be filled out online from the convienience of your computer or hand held device.

One additional Form is required when coming to our office (Not available Electronically).
You can print the
Initial Health Status and bring it in person OR fill out the form at our office.
Note: Additional Forms maybe required if involved in a car accident or getting other services such as Acupuncture.
NEW PATIENTS PAPER FORMS (For Print Only)
Perez Chiropractic offers most of our required forms online. These online forms can be filled out at your home or office. Please complete and bring these forms to your 1st appointment. We can also send the signature forms to an email or (see above link) and you can sign electronically to cut down on the time you spend filling out paperwork in our office.
If unable to complete forms at home/office you are encouraged to arrive 15 min prior to your appointment to fill out and sign all nessessary forms. Uncompleted forms at the time of your appointment could cause delay in your sceduled appointment time.
